Position Details

Job Title: Support Coordinator
Program: Developmental Disabilities Division (DDD)
Reports To: Supervisor/Manager, DDD Program
Location: Hybrid Remote – Newark, NJ 07103
Support Coordination Service Area: Atlantic, Burlington, Camden, Somerset & Gloucester Counties
Pay Rate: Per Diem – TBD
Shift: Daytime

What We're Looking For

The Support Coordinator provides essential administrative and operational support within the DDD program. This role assists with the coordination of services for adults with developmental and/or intellectual disabilities (DD/ID).

The Support Coordinator is responsible for assisting with documentation, scheduling, communication, monitoring, and follow-up to help ensure that individuals receive timely and appropriate services.

This position is ideal for someone who wants to gain hands-on experience in the DD/ID field while maintaining accurate, organized, and audit-ready documentation.

What You'll Do

1. Administrative Support

  • Assist with managing caseloads for adults with DD/ID.

  • Maintain organized client records.

  • Schedule appointments and coordinate meetings with clients and service providers.

  • Prepare documentation for Individualized Service Plans (ISPs), progress reports, and compliance audits.

2. Data Entry & Documentation

  • Accurately input and update client information in DDD-approved databases.

  • Maintain accurate records of services provided, including dates, hours per week, and length of service.

  • Ensure documentation is complete, organized, and up to date.

3. Client & Provider Communication

  • Serve as a point of contact for clients and families.

  • Answer general questions and provide information regarding available services.

  • Assist with scheduling and follow-up.

  • Communicate with service providers to ensure they have the necessary documentation for timely service delivery.

4. Service Coordination Assistance

  • Assist with coordinating services across healthcare, vocational, residential, and community support settings.

  • Track service implementation and help ensure services align with Individualized Service Plans (ISPs).

5. Monitoring & Follow-Up

  • Assist with monitoring client progress toward established goals.

  • Gather information and data for reporting purposes.

  • Follow up with clients, families, and providers to ensure services are being delivered as planned and in accordance with DDD requirements.

6. Training & Resources

  • Assist in distributing training materials and resources to clients and families.

  • Help clients and families understand available services, resources, and their rights.

7. Crisis Assistance

  • Assist with organizing resources during crisis situations.

  • Contact appropriate providers and assist with scheduling emergency services when needed.

8. Compliance Support

  • Ensure documentation meets DDD standards and is maintained in an audit-ready manner.

  • Maintain clear documentation regarding the DD/ID population served and the services and hours provided.

The Ideal Candidate

Required / Preferred Qualifications

  • Bachelor’s degree in Human Services, Social Work, Psychology, or a related field preferred.

  • Experience in administrative, customer service, or social services settings preferred.

  • Experience working with adults with DD/ID is highly preferred.

  • Strong organizational skills and attention to detail.

  • Ability to manage multiple priorities and meet deadlines.

  • Excellent written and verbal communication skills.

  • Proficiency with Microsoft Office Suite and database systems.

  • Ability to maintain confidentiality and comply with HIPAA and applicable privacy requirements.

Additional Preferred Qualifications

  • Experience working with individuals with developmental or intellectual disabilities.

  • Knowledge of DDD services, regulations, and community resources.

  • Bilingual skills, including Spanish, ASL, or other languages.

DDD Compliance Note

To support eligibility and DDD audit requirements, employment history should clearly document relevant experience, including:

  • Direct work with adults with developmental and/or intellectual disabilities.

  • Number of hours worked per day and days worked per week.

  • Total length of service.

  • The organization’s mission and its alignment with DD/ID services.
